While you need not have a title page, the rest of your paper should be in MLA format, with in-text citations throughout, and a suitable bibliography, referred to as Works Cited (for an example, see here).

Note that MLA asks for page numbers for all citations, not just for quotes.
All borrowed information must be cited: facts, biography, ideas, and quotes.

Page numbers should be given for e-documents whenever possible.

URLS never appear in the text or the bibliography.
